| C-TAC and Healthsperien are collaborating with the Gordon & Betty Moore Foundation, the Betty Irene Moore School of Nursing at UC Davis, and the Petrie-Flom Center for Health Law Policy, Biotechnology, and Bioethics at Harvard Law School to develop a flexible serious illness care model implementation framework that will identify the foundational elements of program design and factors that contribute to program success. This presentation will discuss the development of this framework and outline how programs can address the needs of people living with serious illness and their caregivers. Participants will have the opportunity to provide input on the model framework and consider how it relates to different aspects of the healthcare system.
